Our personalized services include the following:
 

3hr consultation (9am -12noon or 1:30pm-4:30pm): $240
This comprehensive consultation includes personalized, private, comprehensive health education.
Health care review, health care coordination and Caregiver relief/respite service.

4hr allergy consultation with menu planning and cooking demo: (9am to 1pm): $320
This 4hr. consultation is designed for the newly diagnosed food-allergic client. The basic components of the consultation include:
- Personalized, private session
- Hands-on menu planning and shopping
- Hands-on meal preparation/demonstration for children and adult clients
- Paperwork completion (school forms etc...)
- Travel planning and assistance
Every client is unique, and this consultation can easily be adjusted to meet your individual needs.

2hr telephone consultation: $160 (evenings only)

This is perfect for those who don't have the time for an in-person consultation or for individuals who live outside of the Jacksonville/St. Augustine Florida area.

New! Respite/Caregiver Relief services: $28/hr. Please call (904)588-2800 for more information and to schedule a day & time.

Disclaimer: Each consultation session will include a period of time where your personal RN completes necessary paperwork for you. During this time your RN will need a small workspace to complete and print out forms for you to keep and give necessary parties like your child’s school nurse etc…Please feel free to take care necessary tasks during this time. During a telephone consultation your RN will be available solely to you via telephone if you need to hang up temporarily during your consultation.

SWN Nurse Consultation company goals

To assist clients in adapting their lifestyle to meet their health needs as recommended by their physician or health care provider. To ensure that the client fully understands their medical diagnoses and the medical terms used by their physician or health care provider.

To ensure that the client is fully knowledgeable about their medications, diet requirements, activity requirements, upcoming surgeries or any other health issues that concern them.

Method

SWN Nurse Consultation meets the unique learning needs of each client by providing every client with private, confidential, completely individualized health education.

Process

Each client that determines that they need the services that SWN Nurse Consultation provides will retain a consultation day with a retainer (PayPal?)via an interactive calendar.

The client will sign a waiver that will simply state that SWN Nurse Consultation does not dispense medical advice, but strives to ensure that the client is successful in adapting their lifestyle to meet the recommendations of their health care provider.

Next, the client will complete a learning need assessment survey that will include a mix of closed and open-end questions.

The information gathered by the learning need assessment survey will be used by the RN to research the client’s medical diagnoses, medications etc... and an individualized teaching plan will be started. 

The RN will meet with the client for a private consultation that will consist of an interview with the goal being to discover all of the clients concerns.  During the interview the teaching plan will be adapted via notebook computer.

The client and RN will prioritize the client’s learning needs, and then each learning need will be addressed to the client’s liking.

After the client/RN meeting, the RN will finalize the teaching document, present the client with a copy, then review the session with the client to ensure all of their learning needs have been met.

The RN and client will both sign the bottom of the teaching document.
